Corticosteroid vs. PRP Injections for Joint Pain: What Is the Difference and Which Lasts Longer?

Joint pain that does not respond to rest and medication leaves many patients wondering what comes next. At Advanced Bone & Joint, two of the most common injection-based treatment options we discuss with patients are corticosteroids and PRP, and understanding how they differ is the first step toward choosing the right one.

How Corticosteroid Injections Work for Joint Pain and When They’re Appropriate

When corticosteroids, a synthetic version of a hormone the body naturally produces, are injected directly into a joint or the surrounding tissue, inflammation subsides quickly and effectively. The relief can begin within a few days and typically lasts anywhere from several weeks to a few months, depending on the patient and the condition being treated.

Corticosteroid injections are best suited for conditions where inflammation is the primary driver of pain. Common candidates include knee osteoarthritis, shoulder bursitis, hip impingement, and flare-ups of chronic joint conditions like rheumatoid arthritis. They are also useful as a bridge treatment, providing enough relief to allow a patient to participate productively in physical therapy.

There are limits to how frequently cortisone can be used. Repeated injections into the same joint over a short period can weaken cartilage and surrounding tissue over time. Most providers recommend no more than three to four cortisone injections per year in a single joint, and the decision to repeat them should always be guided by how well the previous injection worked and how long relief lasted.

What Is PRP Therapy and How Does It Compare to Corticosteroids for Orthopedic Injuries?

Platelet-rich plasma therapy, commonly called PRP, takes a different approach entirely. Rather than reducing inflammation with a synthetic compound, PRP uses a concentration of your own blood platelets to stimulate the body’s natural healing response. A small amount of blood is drawn, processed in a centrifuge to concentrate the platelets, and then injected into the injured area.

The key distinction in the PRP vs. cortisone joint pain conversation is the mechanism. Corticosteroid suppresses inflammation for symptom relief. PRP works to promote tissue repair at the cellular level. This makes PRP a stronger candidate for conditions where the goal is actual healing rather than temporary pain management, such as tendon injuries, partial tears, and early to moderate joint degeneration.

PRP injections for sports injuries are increasingly used for conditions like tennis elbow, patellar tendinopathy, and rotator cuff tendinosis. Recovery after a PRP injection looks different from corticosteroid, as well. Patients often experience a temporary increase in soreness in the days following the injection as the inflammatory healing response is triggered, before improvement gradually sets in over several weeks. There are often strict activity restrictions for the first 4-6 weeks to ensure the PRP works properly.

In terms of duration, PRP tends to offer longer-lasting results than corticosteroids when it works well, with many patients reporting sustained improvement for six months to a year or more. However, it is not universally effective, and results vary based on the condition, the severity of tissue damage, and individual patient factors.

How to Decide Which Injection Is Right for Your Knee, Hip, or Shoulder Pain

Choosing between corticosteroid and PRP is not a one-size-fits-all decision. The right injection depends on your diagnosis, timeline, goals, and, in some cases, your budget, since PRP injections are typically not covered by insurance and are paid out of pocket.

Here is a general framework for how orthopedic providers approach the decision:

Corticosteroid Is Often the Better Starting Point When:

  • Inflammation is the primary cause of your pain
  • You need fast relief to function or participate in therapy
  • You have an upcoming event or obligation that makes a temporary flare of symptoms from PRP impractical
  • Your condition is well-established and the goal is pain management rather than structural repair

PRP Is Worth Considering When:

  • You have a tendon injury or partial soft tissue damage that has not responded to other treatments
  • You are looking for longer-lasting results and are willing to allow more time for the response to develop
  • You have already tried costeroids with diminishing returns
  • You are an active patient or athlete looking to support tissue recovery rather than just manage symptoms
