What Does Pain Feel Like After Knee and Hip Replacement?

October 13, 2026

Every patient asks some version of the same question before surgery: what will pain actually feel like after knee and hip replacement? Understanding the joint replacement pain timeline in advance helps you prepare mentally and physically, and at Advanced Bone & Joint, we walk every patient through what’s normal and what’s not before they ever reach the operating room.

Why Does Knee Replacement Cause Swelling and Bending Discomfort?

Swelling is one of the most common complaints after knee replacement, and it’s a normal part of your body’s healing response. The knee joint has less soft tissue padding than the hip, so fluid tends to collect around it more visibly during the first few weeks.

Bending discomfort is also common, especially first thing in the morning or after sitting for a while. This usually improves as scar tissue softens and the muscles around the knee regain strength.

  • Swelling typically peaks around day 3 to 5 and gradually decreases over 6 to 12 weeks
  • Ice and elevation help manage swelling in the early weeks
  • Bending past 90 degrees often takes several weeks of consistent stretching
  • Persistent swelling that increases suddenly after week 2 should be reported to your surgeon

Why Does Hip Replacement Pain Radiate to the Groin or Thigh?

Hip replacement pain feels different from knee pain because of how the hip joint connects to surrounding muscles and nerves. Many patients recovering from hip replacement notice discomfort that radiates into the groin, outer thigh, or even down toward the knee, rather than staying isolated to the hip itself.

This referred pain pattern is normal and happens because the muscles and nerves around the hip joint share pathways with the thigh and groin. It typically fades as the surrounding muscles heal and regain strength over the first 6 to 8 weeks.

How Is Pain Managed During Physical Therapy After Joint Replacement?

Physical therapy plays a central role in recovery, and some discomfort during sessions is expected as stiff tissue is stretched and strengthened. Your therapist will adjust the intensity of each session based on your pain level and progress.

Common pain management strategies during this phase include:

  • Timing pain medication before therapy sessions for better participation
  • Applying ice after sessions to reduce inflammation
  • Gradually increasing exercise intensity as strength improves
  • Using assistive devices like a walker or cane as recommended

Our team reviews your pain levels at each follow-up visit to make sure your recovery is on track. If pain is not improving or worsens instead of easing, that’s a sign to reach out to your care team rather than waiting it out.

Carol Owen

A+ experience from start to finish. The staff in the waiting area are very capable and personable. The wait time is short and my most recent visit was my third time there with the third specialist. I have been a patient of Dr. Frisella, Dr. Glissom and now Dr. Fallon. I always rave about their knowledge, expertise and care. I recommend them to all my friends, and give them a five star recommendation to all!

Alma Gayle

Everyone was great and Dr Spezia is my kind of Doctor! Thorough, and a great upbeat personality!
